If you require any vaccinations relating to foreign travel you will need to collect a travel vaccination form from reception (or print from link below) and complete, ticking all the vaccinations that you require. We do not advise which vaccinations are needed, you will need to seek this advice from the appropriate agencies.
Please note that we are unable to provide travel vaccinations if your date of departure is in 2 weeks or less. We ask that you give us at least 6 weeks notice before you travel.
The Surgery will contact you after receiving a completed travel vaccination form to arrange a suitable appointment.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
